一、   設置宗旨:任江履昇女士之家屬為回饋社會並鼓勵輔大清寒同

學努力向學,將來服務社會人群,設置「任江履昇

女士清寒奬學金」以永續嘉惠無數莘莘學子。

二、   設置方式:捐款新台幣3,0264,697元(美金100萬元),成

立永續基金專戶,受贈單位每年提撥專戶之4%保證

利息收益作為獎學金,本金永不動用。

三、   獎學金金額:每學年新臺幣6萬元整

四、   獎學金名額:每年20名。

五、   申請時間:每學年第一學期開學註冊後一個半月內向學務處生

活輔導組申請。

六、   申請資格:

(一)本校大學部(二至四年級)在學學生。(研究生限發生重大意外變故者)

(二)家境清寒或家中突遭重大變故者

(三)上一學年學業平均成績75分以上,操行成績80分以上者;有特殊原因,如意外變故,或參加特殊比賽訓練,以致無法達到標準者,得提供具體事證說明,參加申請。

(四)有社團領導經驗,社會服務或工讀證明者為優先。

() 受領本獎學金者,得兼領其他獎學金,其總額30,000元為限。全年全部獎學金NT$90,000為限。

七、   繳交資料:

(一)申請書。

(二)戶籍謄本。

(三)家庭成員每人國稅局年度全戶綜合所得稅各類所得資料清單與家庭成員每人的財產歸屬資料清單。

(四)低收入戶或中低收入戶證明(無者免附,僅供參考)。

(五)上學年成績單(含班級排名,獎懲紀錄)。

(六)家中突遭重大變故者請附說明書,並經導師核章或提具醫        院診斷證明書。

(七)有社團經驗、社會服務或工作經驗之相關證明或獎狀。

(八)詳細自傳、履歷表、正面照片。

(九)導師或教師推薦信1封。

(十)上述申請書、自傳與履歷表(2吋正面脫帽半身照片)文件之電子檔資料。

八、   申請程序:

(一)程序:本獎學金由本校學務處生輔組負責申請、審查事

 宜,將初審結果之推薦學生及備取6名學生之申

 請資料送設獎人複核確定後,再行辦理發放事宜,

 並將印領清冊函送捐贈人代表。

(二)原則:審查以家境貧困、品行端正為主要考量,並輔以學

   業成績做為次要考量。其審核程序宜由權責相關單

   位及導師從嚴辦理。

九、   每年會計年度結束時,學校得提供當年度的本獎學金之會計報表給捐贈人代表。

十、   本校必須於獎學金發布申請前,為此獎學金設立網頁。此網頁由學務處與捐贈人代表共同管理。

十一、  本獎學金申請詳細作法,每兩年校方及捐贈人代表得依助人  需求提出檢討修訂。

十二、  本設置要點經就學獎補助基金委員會議通過即可,修正時    同。


 

Fu Jen Catholic University Guidelines for Mrs. May Jen Scholarship for Low Income Students

Passed by the Executive Council on June 18, 2015

I.                    In order to pay back to the society, and encourage FJCUstudents from low income families to study diligently so as to serve society in the future, the family members of Mrs. May Jen (the donor hereafter) offered this scholarship.

II.                 The donor promises to donate NT$ 30,264,697 (US$ 1,000,000) into the university endowment fund. FJCU guarantees a 4% pay out ratio each year to be used as the scholarships, and the principal fund shall be not mobilized.

III.              The amount of each scholarship: NT$ 60,000 per academic year.

IV.              Number of the awardees: 20 awardees per academic year.

V.                Eligible applicants must submit all application materials to FJCU Student Life Division within one and half months after registration of the first semester.

VI.              Eligibility:

1.

FJCU undergraduate students (The second, third, and forth year students. Graduate students are eligible only when recently experiencing or encountering a major accident or a serious family disaster).

2.

Students from low-income families or students who recently encountered an accident or a facility disaster.

3.

Students whose grades average above 75 in their academic and the above 80 in conduct performance from the previous academic year. When unable to meet the above standards, applicants may offer evidence to explain the reasons why they could not meet the standards, such as accidents or participation in contest training.

4.

Students who have experience of leading student organizations, participating in social services or working part-time will be given priority.

5.

Students who are granted this scholarship can have other scholarships, but the amount is limited to NT$ 30,000, that is, the total amount of all scholarships for an individual awardee shall not exceed NT$ 90,000.

VII.           Application Materials:

1.

Application form.

2.

Household registration transcript.

3.

Annual income tax report and individual property certificate of all the members of the family.

4.

Low or middle-low income household certificate. (For reference. Not required).

5.

Transcript from the previous academic year (with class ranking and record for commendation and recrimination.).

6.

Applicants who recently encountered an accident or a family disaster should provide a statement verified by the their class advisors or a certificate issued by the hospital.

7.

Related proof documents of student societies leadership or working experience and social services.

8.

A detailed autobiography, CV, and a picture.

9.

A recommendation letter written by the class advisor or a faculty member.

10.

The digital files of the above application form, autobiography and CV (including a picture). 

VIII.        Application Procedure:

1.

Procedures: FJCU Student Life Division is responsible for receiving the applications, reviewing and submitting the list of recommended applicants and 6 applicants on the waiting list for the approval of the donor. After the approval of the donor, FJCU Student Life Division grants the scholarship to the awardees, and sends the scholarship receiving list to the donor representative.

2.

Principle: The scholarship aims to help students of good character who come from low income families, so academic performance is but a supplementary criteria. The review shall be conducted carefully by the authorities in charge and the advisors.

IX.              At the end of each fiscal year, FJCU should offer the accounting statement of the scholarship to the donor representative.

X.                FJCU must set a website for the scholarship before the announcement of application. The website is jointly administered by FJCU Office of Student Affairs and the donor representative.

XI.              The guidelines can be amended by FJCU and the donor representative every two years.

XII.           The guidelines take effect after the approval of the Meeting of Student Subsidy and Scholarship Funding Committee. Amendments to the guidelines should follow the same procedure.
